Documents to be sent for FMS Application - FMS Admission Form

Below is the list of documents which need to be sent to FMS Registrar office:
  • The FMS application form printout taken should be affixed with the candidate’s photograph and duly signed . 
  • The documents of proof for reserved category (if any, refer below links to download the caste certificate proforma for FMS). 
  • The Demand Draft (if payment is not made online and made through DD) .
  • Certificate from the  Principal/HoD/Registrar/Director if candidates appearing in  the  final year of Graduation and also by candidates who have completed the final year but are awaiting the results.(refer below links to download the Final Year Certificate proforma for FMS)
  • An additional passport-size colour photograph.

Note: Though "Candidate Identification Affidavit" form is available on the 
website but its not required to be submitted along with application form.



Address where form should be sent: The Assistant Registrar, Faculty of 
Management Studies, University of Delhi, Delhi-110007, on or before 30th 
November 2011.
